Interior Community Health Center (ICHC) is a Federally Qualified Health Center (FQHC) located in Fairbanks, Alaska. ICHC is requesting proposals for a remote patient monitoring (RPM) nursing service who will provide ICHC's patients with uncontrolled hypertension (approximately 160 patients) the following: cellular enabled blood pressure cuffs, blood pressure monitoring linked to ICHC's Electronic Health Record (EHR), Athena, and individualized health promotion coaching intervention. Scope of Work The selected vendor will be responsible for the following: 1) Provide and integrate RPM software into Athena. 2) Assistance with enrolling ICHC's uncontrolled hypertension patients into its RPM software through December 31, 2026, to include mailing of cellular-enabled blood pressure cuffs directly to enrolled patients and providing assistance to the patient to ensure BP cuff is linked to the patient's EHR. 3) Initiate monitoring and individualized health coaching of hypertensive patients enrolled in RPM through June 30, 2027. Additional requirements: During the contract, ICHC expects the contractor to provide timely communication and reports showing interaction details (date and time) of RPM nursing coaching intervention to individual patients. The blood pressure readings must be linked to the patient's chart in Athena for ICHC's providers to view BP readings seamlessly. Agreement between ICHC and the approved RPM vendor must be completed by 6/30/27. References and samples of BP cuffs and nursing coaching report are required.
