I. PURPOSE Interior Community Health (ICHC) seeks proposals from qualified consultants to assist in the planning, development, implementation, and optimization of a 340B Program and pharmacy services strategy. The goal is to have a contract with a community pharmacy(ies), as well as with an on-site remote pharmacy (with a limited medication formulary). The selected Respondent will provide expertise in 340B compliance, pharmacy operations, Alaska pharmacy law, financial modeling, business planning, dispensing operations, and implementation support. II. ABOUT ICHC Interior Community Health Center is a mission-driven health center located in Fairbanks, Alaska. We provide medical, integrated behavioral health, and dental services to all community members. All of the providers and staff at ICHC focus on delivering patient-centered, quality care "with a heart." Our mission is to provide universal access to excellent primary health care for patients of all ages, stages of life, and backgrounds. ICHC takes a comprehensive approach to health and to long-term relationships with patients to better fulfill their needs. Our providers focus on the whole person and comprehensive care. Our patients have access to a wide range of services, including preventative medicine, wellness services, acute care, chronic disease management, prenatal care, annual exams, short-term health education, promotion and counseling, dental cleanings and treatments, and more. Eighty-nine percent of patients of known incomes are on ICHC's sliding fee. As part of ICHC's commitment to providing comprehensive care to the whole person, ICHC has decided to move forward with establishing pharmacy services for its patients and expand its medication physician dispensing site 340B pharmacy program. Due to the high number of patients who have low incomes, even those with insurance coverage are unable to afford their medications. We invite your organization to respond to this Request for Proposal (RFP) if you are interested in partnering with ICHC for this pharmacy/340B buildout project. Details on responses, evaluation criteria, and proposed timeline are included below. III. SCOPE OF SERVICES • Strategic assessment and pharmacy feasibility study, to include offsite community pharmaceutical services and on-site remote pharmacy dispensing unit operation (Fairbanks location) and assessment of pricing of, contracting for, and maintenance of dispensing unit. This includes negotiation and coordination with the onsite contracted pharmacy for the dispensing unit and method of delivery to the patient (i.e. patient facing or use of pharmacy technician). • 340B program design, registration, and compliance planning. * This includes: i. Review of ICHC data to include prescription-written costs of medication, dispensing fee and patient payment to determine whether it's financially viable; ii. Conduct a sustainability assessment as to ICHC's 340B program and expansion; iii. Development of a strategic business plan for 340B expansion a. Including support with negotiation and partnership with local pharmacies and/or onsite pharmacy. iv. Compliance Review of ICHC's 340B program and buildout of internal 340B compliance processes (i.e., 340B Policy Manual; Pharmacy Manual, annual audit plan); * Alaska pharmacy law and regulatory analysis; * Pharmacy operational planning, staffing, workflows, inventory, and technology; * Financial modeling, reimbursement analysis, pricing strategy, and sustainability planning; * Implementation support and launch readiness. * Remote pharmacy dispensing equipment research. IV. REQUIRED DELIVERABLES • Pharmacy feasibility and sustainability assessment. • Five-year pharmacy strategic business plan. • Five-year financial pro forma. • Sensitivity analyses for prescription volume, payer mix, reimbursement, staffing costs, 340B savings, contract pharmacy arrangements, and break-even projections. • Pharmacy operating model recommendations. • 340B implementation roadmap. • Compliance risk assessment. • Draft pharmacy and 340B policies and procedures. • Implementation timeline and governance structure. • Executive and Board presentation materials. V. MINIMUM RESPONDENT QUALIFICATIONS • Demonstrated at least 5 years' experience implementing 340B and pharmacy programs for FQHCs. • Experience establishing new pharmacy operations. • Knowledge of HRSA 340B requirements and audit readiness. • Familiarity with pharmaceutical company 340B environment changes. • Familiarity with Alaska pharmacy laws and regulations (preferred). • Experience serving rural, frontier, tribal, or safety-net providers (preferred). • Expertise in pharmacy reimbursement, pricing, and dispensing operations. VI. PROPOSAL REQUIREMENTS 1. Firm profile and organizational structure. 2. Relevant project experience with minimum 3 references. 3. Key personnel resumes and qualifications. 4. Alaska-specific experience. 5. Project methodology and work plan. 6. Proposed timeline. 7. Detailed fee proposal. 8. Identification of subcontractors. 9. Description of quality assurance measures. VII. EVALUATION CRITERIA • Relevant FQHC and 340B experience (30%) • Alaska and rural healthcare experience (20%) • Technical approach and methodology (20%) • Qualifications of assigned personnel (15%) • Cost and value (10%) • References and past performance (5%) VIII. MANDATORY BIDDER CERTIFICATIONS Respondent shall certify that: • It is legally authorized to conduct business. • It has not been excluded from participation in federal healthcare programs. • It maintain all required professional licenses. • It will comply with all federal, state, and local laws. • It will maintain confidentiality of protected health information. IX. CONFLICT OF INTEREST DISCLOSURE Respondent shall disclose any actual or potential conflicts of interest, including relationships with pharmacy vendors, wholesalers, software vendors, contract pharmacies, manufacturers, or other entities that may affect the respondent's independence. X. INSURANCE REQUIREMENTS Prior to contract execution, the selected Respondent shall provide evidence of: • Commercial General Liability Insurance. • Professional Liability / Errors and Omissions Insurance. • Cyber Liability Insurance. • Workers' Compensation Insurance as required by law. XI. COMPLIANCE REPRESENTATIONS The Respondent shall represent and warrant that: • Recommendations will be designed to support compliance with applicable 340B requirements. • Services will be performed consistent with industry standards. • Respondent will promptly disclose any known compliance concerns identified during the engagement. XII. Non-Exclusion Certification and Screening Requirements By submitting a proposal, Respondent represents and warrants that neither it, nor any of its owners, officers, directors, employees, agents, consultants, or subcontractors who may perform services under any resulting agreement, is currently excluded, debarred, suspended, or otherwise ineligible to participate in any federal or state healthcare program, including Medicare and Medicaid. Respondent further certifies that it is not listed on the U.S. Department of Health and Human Services Office of Inspector General's List of Excluded Individuals and Entities (LEIE), the General Services Administration's System for Award Management (SAM) exclusion records, or any similar state exclusion database applicable to the services to be provided. Respondent shall conduct exclusion screening of all personnel assigned to perform services under the agreement prior to commencement of services and no less frequently than annually thereafter during the term of the engagement. Upon request, Respondent shall provide documentation evidencing such screening. Respondent shall immediately notify the Health Center if Respondent or any assigned individual becomes subject to an exclusion, debarment, suspension, sanction, investigation, or other action that could affect eligibility to participate in federal or state healthcare programs. ICHC shall have the right to require removal of any such individual and may immediately terminate the agreement for cause if Respondent or assigned personnel become excluded or otherwise ineligible to participate in federal healthcare programs. XIII. PROCUREMENT CONDITIONS ICHC reserves the right to: • Reject any or all proposals. • Request clarifications or additional information. • Negotiate with one or more respondents. • Cancel or reissue this RFP. • Award a contract in whole or in part.
